Non-Prescribed Medication Form
Non prescribed medication (e.g. Calpol) can only be administered in exceptional circumstances where a medical professional has recommended that it should be taken at regular intervals on a short term basis.
A form should be completed each day you require the medication to be administered and should be collected at the end of the school day from the Office or the After School Club if your child is attending that day.
